Forms：比 request form 更靈活的表單

建立Space settings → Request management → Forms：從 200+ 個 template 或 blank 開始；要 Jira admin 或 project admin。
排版Confluence 風格編輯器：標題、bullets、表格、引用、多欄 layout。
Sections＋條件邏輯依 radio、checkbox、dropdown 的答案顯示或隱藏 section；必填欄位不能放在條件 section。
Linked Jira fieldsForm field 只存在表單裡；連到 Jira field 才能被 JQL、automation、報表使用。
放在哪Attach to request types、shareable link、加到既有 issue（含 automation 的 Attach forms）。

建立

建立表單、section 與條件

新表單Forms → 選 template 或 Blank form；有多個 issue type 時先選這張表單要建立的 work 類型。
加內容從右側拖拉欄位；輸入 / 加元素（標題、表格、layout、bullets、quote）。
Section把相關欄位包成 section；選 section → Create logic → Show／Hide → 條件（field、operator、value），可用 AND、OR。條件欄位要是 radio、checkbox、dropdown。
欄位行為每個欄位可設必填、驗證、說明、連到 Jira field；考綱另列 default response（預設回答），官方文件未詳述操作。
Preview測試版面、條件與驗證，再 Save changes。編輯不會影響已經在 issue 上的表單實例。

依「新筆電／維修」顯示不同 section。

Create or edit a formIf your space has more than one work type, you’ll be prompted to select the type of work.

表單狀態	意思
Open for edits	在 customer 手上填寫或修改，可存草稿
Submitted	已送出；agent 可 Edit 或 Open customer edits
Locked	只能檢視；只有 admin 能改

翻譯：表單 Translate 分頁 → Add languages → 逐欄翻譯 → 開 Available to customers。沒有對應語言的 customer 看到 primary language。Portal 的 Language support 也要啟用該語言，否則不會出現。連到 Jira field 的選項欄位目前不能翻譯。

實作 Lab：做一張會變形的表單，掛到 request type

約 25 分鐘HELP 的 project admin

第一次做 Lab？先看 Lab 環境準備：用測試站台、準備測試帳號。畫面名稱可能隨 Atlassian 更新而不同，以下以現行畫面為準。

目標：用 form builder 做一張有條件 section 的表單，附到 request type，再看送出後的鎖定與 linked Jira field 的行為。

這個 Lab 對應的考點

條件 section 依 radio、checkbox、dropdown；必填欄位不能放在條件 section；連到 Jira field 才能被 JQL、automation、報表使用；Lock＝送出後只有 admin 能改；forms 不能用在 email、chat、widget、virtual agent。

準備

沿用 420.4.1 的 Lab 借用投影機（沒做的話任選一個 request type）。

步驟

Space settings›Request management›Forms：選 Blank form（有多個 work type 時先選要建立的 work 類型），標題改成 Lab 筆電申請。
你應該看到：進入 form builder，右側是可拖拉的欄位與元素；輸入 / 可以插入標題、表格、layout。
從右側拖入一個 Radio 欄位「你需要什麼？」，選項 新筆電、維修。再加一個 Section，在 section 裡放一個 dropdown「型號」與一個 text「預計到職日」（都不要設必填）。
你應該看到：表單上出現一個框起來的 section，裡面有兩個欄位。
選這個 section → 右側 Create logic → Visibility 選 Show → Field 選「你需要什麼？」、Operator 選 is equal to、Value 選「新筆電」→ Save。按 Preview 切換兩個選項。
你應該看到：選「新筆電」時 section 出現，選「維修」時消失。條件只能拿 radio、checkbox、dropdown 當依據。
Settings：打開 Attach to request types，選 Lab 借用投影機；Preferences 勾 Lock this form once submitted → Save changes。
你應該看到：Lab 借用投影機的 request form 預覽，下方多了這張表單。
用 Cara 在 portal 提一張 Lab 借用投影機，選「新筆電」，填完送出。
你應該看到：表單接在 Jira 欄位下方，看起來像同一張表；送出後在 request 上另有 Forms 區塊。
用 Leo 打開這張 request 的 Forms 區塊；再用你自己（project admin）打開。
你應該看到：表單是鎖定狀態：Leo 只能檢視；你看得到 Edit 或 Open customer edits。
回到 form builder，把「預計到職日」改名成「到職日期」→ Save changes。重新整理 Cara 那張 request。
你應該看到：既有 request 上的表單不變：編輯只影響之後的新實例。

試試看：改一個設定，觀察差異

T1在條件 section 裡選「型號」，試著把它設成必填。看結果

做不到或會被擋：必填欄位（包括連到 Summary 的欄位）不能放在條件 section。要必填，就移到 section 外。

T2在表單外加一個 text 欄位，右側 Linked Jira field 選 Description → Save changes，再看 Lab 借用投影機的 request form。看結果

Description 從 request form 被移除，改由表單欄位填：同一個 Jira field 不能同時出現在 request form 與表單上。做完取消連結，並把 Description 加回 request form。

T3（想像，不實際做）把 Lab 借用投影機設成某個 email address 的 request type，customer 用 email 提單。看結果

Email 建的 request 不會帶這張表單：forms 目前不能用在 email、chat、virtual agent、widget。

心像錨點：閉上眼睛想一次這個畫面

Form builder 裡那個 section：右側面板寫著 Show when「你需要什麼？」is equal to 新筆電。條件看選項，連了才算數，鎖了只剩 admin。

收尾

Lab 筆電申請的 Settings → 關掉 Attach to request types。
確認 Description 仍在 Lab 借用投影機的 request form 上。
